This 85 years old male presented with cough and breathlessness on exertion and was evaluated outside with CT scan which showed large right parahilar mass+ Right lower lobe mass with pleural effusion.Bronchoscopic biopsy was suggestive of Moderately differentiated #adenocarcinoma of #Lung.We sent the biopsy block for EGFR/ALK/ROS mutation testing.Report of EGFR mutation came positive for Exon 21 mutation in next 5 days.Patient was then started on only oral targeted therapy Tablet Afatinib 40 mg once a day.Post 3 months of Afatinib patient has almost complete resolution of symptoms and has great response on PETCT scan.
